I need a form to put on a jobsite to indicate frequency and technician to be placed at each piece of equipment to track service.

I have attached a tag I have used at customer locations in the past.

This is a crude copy that I sent to a tag maker. The tags were already cut to size (with a hole to zip strip) and we apply a clear label (supplied from tag maker) with place to put the equipment number and the correct lubricant for each lubrication point.
