Patch test booklet (NAS1638 / AS4059)

Anyone know of a supplier of a patch test booklet type of product for onsite cleanliness identification to use together with microscope. I know Millipore has one, but it appears to be in the NAVAL cleanliness system and not NAS1638 or SAE AS4059.
